Some moments don’t look dangerous at all when they happen. A casual evening with friends, a joke that goes a little too far, a split-second decision that feels harmless in the moment. And yet, sometimes, those exact moments become life-changing turning points—precisely because no one sees the risk coming.
In 2010, in Australia, a healthy and athletic 19-year-old named Sam Ballard found himself in that kind of situation. Surrounded by friends, in a relaxed setting, he accepted what seemed like a harmless dare: he swallowed a garden slug.
What followed wasn’t immediate or dramatic. There was no instant reaction, no obvious sign that something had gone wrong. Instead, the real process had already begun—quietly, inside his body.
What actually enters the body
Slugs can carry a parasite known as Angiostrongylus cantonensis, often referred to as “rat lungworm.” In its natural life cycle, this parasite lives in rats, while snails and slugs act as intermediate hosts.

Humans are not meant to be part of this cycle. However, in rare cases—such as accidental or intentional ingestion—the parasite’s larvae can enter the human digestive system.
At this stage, nothing feels dramatic. There are no immediate warning signs. But internally, the body is now dealing with something it was never designed to handle.
The first phase: silent entry
Once inside the body, the larvae pass through the stomach and reach the intestines. In many cases, the body’s immune system is able to stop the infection early, which is why severe outcomes are rare.
But in some situations, the larvae manage to cross the intestinal wall and enter the bloodstream. From that moment on, the situation is no longer limited to the digestive system—it becomes systemic.
Early symptoms can be vague: fatigue, mild headaches, a general feeling of being unwell. These are easy to dismiss, which is part of what makes the condition so deceptive.
Migration to the brain
What makes this parasite particularly dangerous is its ability to travel to the central nervous system. The larvae can migrate toward the brain, where they trigger a strong inflammatory response.
The immune system reacts not only to the presence of the parasite, but also to the damage it causes. This leads to a condition known as eosinophilic meningitis, a type of inflammation affecting the brain and its surrounding membranes.
At this point, the problem is no longer hidden. The body is actively struggling to respond, and the effects begin to surface more clearly.
What happens inside the brain
As inflammation builds, symptoms become more severe. People may experience intense headaches, neck stiffness, nausea, sensitivity to light, and in some cases, neurological impairment.
The brain is extremely sensitive to pressure and inflammation. As these processes intensify, normal function becomes disrupted. Communication between neurons is affected, and basic functions such as movement, coordination, and awareness can begin to deteriorate.
In Sam Ballard’s case, the progression was rapid and devastating. The infection caused significant brain damage, and he eventually fell into a coma.
After the acute phase
Although he survived the initial stage, the neurological damage was severe and irreversible. Sam was left paralyzed and required full-time care.
His life did not return to what it had been. Instead of recovery in the traditional sense, the years that followed were about long-term care and adaptation to a profoundly changed reality.
He lived for approximately eight more years after the incident, supported by his family. In 2018, at the age of 28, he passed away due to complications related to the condition.
How rare is this—and why it still matters
Infections with Angiostrongylus cantonensis in humans are considered rare, and most cases occur in specific regions, including parts of Asia and Australia. Not every exposure leads to illness, and severe outcomes are uncommon.
However, the medical mechanism is well documented: ingestion, larval migration, immune response, and potential neurological involvement.
Basic prevention remains straightforward—avoiding the consumption of raw snails or slugs, thoroughly washing produce, and being mindful of potential contamination.
A moment that changed everything
What makes this case so difficult to ignore is not just its rarity, but how it began. There was no complex medical background, no underlying condition, no obvious danger.
It started with a single, seemingly insignificant decision.
The human body is remarkably resilient, but it is not invincible. Sometimes, the greatest risks don’t come from situations that look dangerous—but from those that don’t look like anything at all.
